Preadmission may be handled in 3 easy steps:

Step 1:

Preadmission forms are available either in your obstetrician's office or at the hospital. Please complete and return no later than fifth month of pregnancy. After we receive your form, we will verify your insurance benefits to determine any deductibles and co-pays for which you are responsible.

Step 2:

Stop by the hospital Maternity Admissions Office to sign admission forms, complete medical questionnaire and assign insurance benefits. To prevent identity theft, please bring a driver's license or other picture ID and your insurance card. This will take about 10 minutes of your time. You may pre-register Mon - Fri: 7:30am - 4:00pm.

Step 3:

Please plan to pay any portions due by your seventh month of pregnancy. You may pay online, over the phone, by mail, or stop by the Maternity Admissions Office.


With the pre-admission information properly completed, you will be asked to proceed to the OB Assessment Center located near the Baby Suites on the third floor of our Women's Center.
